The Anthem CDHP medical plan option offers the lowest per-paycheck cost and includes a Health Savings Account to create a reserve of funds for out-of-pocket expenses. You’ll pay 100% of negotiated costs until you meet your deductible, after which you pay a 20% cost share for in-network care up to the out-of-pocket maximum. This plan includes coverage for out-of-network doctors, though you’ll typically spend less by staying in-network.
The Anthem Low Deductible PPO medical plan option features a low deductible, meaning you pay less before the plan begins to pay a portion of costs. After meeting the deductible, you pay 20% for most services up to the out-of-pocket maximum. This plan includes coverage for out-of-network doctors, though you’ll typically spend less by staying in-network.
The Anthem EPO medical plan option features predictable copay-based fees for office visits with no deductible to meet. This plan has the highest per-paycheck cost and does not include coverage for out-of-network doctors.
An HMO gives you more predictable costs but less flexibility. There is no deductible and you pay a copay for most services. All care must be received within the Kaiser network, which allows Kaiser to coordinate your entire care experience. This plan does not include coverage for out-of-network doctors.
